I have taken it and I will always follow it. I will always maintain the dignity and reputation of the institution. Acknowledgments I will not commit any wrongdoing against the service organization and if I do so, the organization will always be free to take legal action against me. I will keep giving complete details of the work done by me to this organization from time to time. There is no criminal case registered against me. The jurisdiction of all disputes will be Hanumangarh. I have read, heard and understood all the terms and conditions of the organization. Therefore, I request you to accept my application and allow me to work in the organization as soon as possible.

भारत का एक आम नागरिक हूं। मैं ईश्वर को साक्षी मानकर यह शपथ लेता हूं/लेती हूं कि मैनें संस्था, आभार सेवा संस्थान की सदस्यता एक स्वयं सेवक के रूप में अपनी मर्जी से ग्रहण कर रहा हूं/ रही हूं मैनें संस्था के सविधान को अच्छी तरह से पढ़/सुन व समझ लिया है तथा मैं सदैव इसका पालन करूंगा/करूंगी। मैं संस्था की गरीमा को और प्रतिष्ठा को सदा बनाये रखूंगा/ रखूंगी। आभार सेवा संस्थान के विरुध कोई गलत कार्य नहीं करूंगा करूंगी और अगर मैनें ऐसा कियातो मेरे ऊपर कानूनी कार्यवाही करने के लिए संस्था सदा स्वतंत्र होगी मैं इस संस्था को समय-समय पर मेरे द्वारा किये गये कार्य का पूर्ण विवरण देता रहूंगा/ रहूंगी। मेरे ऊपर किसी भी प्रकार का आपराधिक मुकदमा दर्ज नहीं है। समस्त विवादों का न्याय क्षेत्र हनुमानगढ़ होगा। मेरे द्वारा संस्था के समस्त नियम व शर्ते पढ़, सुन व समझ ली है। अतः आपसे प्रार्थना है कि मेरा आवेदन स्वीकार किया जाए एवं संस्था में अतिशीघ्र कार्य करने की अनुमति दी जाए।
